Why Perplexity Cites Review Sites, Not Your Medspa

To get recommended on Perplexity as a local business, stop pouring effort into your own homepage and start shoring up the third-party pages Perplexity already trusts: your Yelp listing, RealSelf profile, Google Business reviews, and the local “best medspa in [city]” roundups. Perplexity builds a local answer by pulling short, quotable lines from a handful of independent sources, then footnoting them, and your self-published marketing copy almost never makes that cut.

I run Sprout Sage Solutions, and I test this constantly with medspa and aesthetic-clinic clients. When I ask Perplexity “best medspa for Botox in Scottsdale” or “where to get lip filler near me,” the citation strip at the top is dominated by Yelp, RealSelf, Healthgrades, and a couple of local blog roundups. The clinic’s own site, if it appears at all, is usually citation four or five, or missing entirely. That is not a bug you fix on your website. It is a signal about where Perplexity looks, and it tells you exactly where to put your work.

Why Perplexity trusts the review site over your homepage

Perplexity is a retrieval-and-synthesis engine. For a local question it fetches a set of candidate pages, ranks them for relevance and trust, extracts the sentences that directly answer the query, and cites the source. Three things about that process push your homepage down the list.

First, independence. Your homepage says you are the best. A Yelp page with 214 reviews and a 4.7 average says it for you, and Perplexity treats third-party corroboration as more reliable than self-claims. This is the same E-E-A-T logic Google uses, applied to a live answer.

Second, structure. Review sites publish in a format the model loves: a business name, a category, a star rating, a review count, an address, and dozens of specific, named-treatment sentences (“I got morpheus8 here and the tightening was noticeable in three weeks”). That is dense, extractable, attributable text. Your hero section that reads “Rediscover your radiance” gives the model nothing to quote.

Third, freshness and volume. RealSelf and Yelp pages update every time a customer posts. Your homepage might not have changed in eight months. Perplexity favors sources that look actively maintained for the exact entity being asked about.

So the honest answer to “how do I get recommended on Perplexity” is: you influence the sources Perplexity already cites, and you make your own pages so clean and factual that they earn a spot beside them. Step 1: Find out which sources Perplexity is actually citing for you

Do not guess. Run the real queries. Open Perplexity and run the 8-12 questions a prospect would actually type for your clinic. Use this pattern:

  • “best medspa in [your city]”
  • “where to get [your top treatment] near [neighborhood]”
  • “[your city] botox reviews”
  • “is [your clinic name] good”
  • “top rated [treatment] clinics [city]”

For each answer, write down every cited domain in the sources strip. After ten queries you will have a frequency table. In every medspa audit I have run, the same five or six domains repeat: Yelp, RealSelf, Google (via the Maps pack), Healthgrades or Vagaro, a city “best of” listicle, and sometimes an Instagram or a news mention. Those repeat-citation domains are your seeding targets, ranked by how often they appear. That frequency count is your priority order — shore up the page cited six times before the one cited once.

Step 2: Shore up the pages in priority order

Now you fix the actual pages Perplexity pulls from. Here is the order I work in and what “shored up” means for each.

Google Business Profile (fix first, always)

This feeds the Maps pack that Perplexity leans on for “near me” answers. Get the category exact (Medical spa, not Spa), fill every service with its real name (Botox, Dysport, Morpheus8, hydrafacial), and get the review count moving. A profile with 40 reviews loses to one with 180 every time. The single highest-return action for most clinics: a post-visit text that asks the happy client to name the treatment in their review. “Great experience” helps you rank; “the Dysport for my forehead lines settled perfectly by day 5” gets quoted verbatim in the answer.

Yelp and RealSelf

RealSelf is disproportionately powerful for injectables and devices because Perplexity treats it as category-authoritative. Claim the profile, complete every field, add real before/after photos with treatment labels, and respond to reviews using the treatment name in your reply — your responses are indexed text too. On Yelp, the same review-language play applies: the specific, named-treatment reviews are the ones that get extracted.

The local “best of” roundups

Search “best medspa [city]” yourself and note which blogs and local magazines rank on page one. Those are the exact roundups Perplexity cites. Getting listed is old-fashioned outreach: email the author with a genuine reason you belong on the list (a specialty, an award, a device no one else in town has). One inclusion on a roundup Perplexity already cites can do more for your AI visibility than a month of homepage edits.

Directories and booking platforms

Vagaro, Healthgrades, Zocdoc, and similar. Consistency of your name, address, and phone across all of them matters, because contradictory data makes the model less confident and less likely to cite any of them for you.

Step 3: Write the citable phrasing that gets pulled into the answer

This is the part most owners miss. Perplexity extracts sentences, so you want your factual claims written as clean, self-contained, quotable statements — everywhere you control the text (your GBP description, your RealSelf bio, your review responses, your service pages).

The formula I use: specific claim + entity + qualifier + concrete detail.

Weak, un-citable: “We offer the best skin tightening in the area.”

Citable: “Sprout Sage-partnered clinics in Scottsdale offer Morpheus8 RF microneedling, typically over 3 sessions spaced 4 weeks apart, for skin tightening on the face and neck.”

Notice what makes the second one work: a named treatment, a named place, a real protocol, no adjectives the model has to trust on faith. That sentence can be lifted whole into an answer with a footnote. Write your GBP services description and your directory bios this way. Give every claim a number, a name, or a location. Strip the “radiance” language — it is invisible to a retrieval engine.

One more: add a short, plain-language FAQ to your own service pages using real questions (“How much does Botox cost in [city]?” “How long does Morpheus8 take to work?”) and answer in two factual sentences. Perplexity will sometimes cite your page for a sub-question even when the roundups win the headline query — and those answers are the easiest text on your whole site to get quoted.

What order to do this in this week

If you do nothing else: run the ten queries (30 minutes), build your citation frequency table, fix your Google Business Profile categories and services (1 hour), and launch a post-visit review text that asks clients to name their treatment. That combination moves the needle fastest because it strengthens the two sources — Google and your review count — that nearly every local Perplexity answer leans on.
