Why this matters more than your Google rank
A growing share of people looking for Botox, filler, laser hair removal, or a “good medspa near me” now ask an AI assistant first and a search engine second. When someone types “best medspa in Scottsdale for lip filler” into ChatGPT, they get a short list of two to five clinics, not ten blue links. If you’re not on that short list, you’re invisible to that buyer before they ever see your website.
The tricky part: these answers are personalized, cached, and constantly changing. You cannot assume you’re recommended just because you rank on Google. AI models pull from reviews, directory listings, your site content, and third-party “best of” articles in ways that don’t map cleanly to SEO. The only reliable way to know your status is to test it directly, the way a prospective client would.
The 12-prompt test script
Run each of these prompts fresh in ChatGPT, Perplexity, and Google (watch for the AI Overview at the top). Replace the brackets with your city, neighborhood, and real treatments. Use a logged-out or incognito session where you can, so personalization doesn’t flatter your results.
General discovery (prompts 1-4)
- “What’s the best medspa in [city]?”
- “Best medspa near me in [neighborhood], [city].”
- “Where should I go for aesthetic treatments in [city]?”
- “Recommend a few top-rated medspas in [city] with good reviews.”
Treatment-specific (prompts 5-9)
- “Best place for Botox in [city]?”
- “Where can I get lip filler in [city] from an experienced injector?”
- “Top clinics for laser hair removal in [city]?”
- “Who does the best [your signature treatment, e.g. morpheus8 / hydrafacial / CoolSculpting] in [city]?”
- “Affordable but reputable medspa in [city] for [treatment]?”
Comparison and trust (prompts 10-12)
- “Compare the top 3 medspas in [city] for [treatment].”
- “Is [Your Medspa Name] a good place for [treatment]?”
- “What do reviews say about [Your Medspa Name]?”
That’s 12 prompts across 3 platforms, so 36 total checks. It takes me about 45 minutes per clinic. Prompts 1-10 test whether AI surfaces you unprompted. Prompts 11-12 test what AI says once your name is in play, which reveals whether your reputation data is accurate or stale.
The scoring sheet
For each of the 36 checks, score three things:
- Presence (0 or 2): 2 points if your medspa is named in the answer, 0 if it’s absent.
- Rank (0, 1, or 2): 2 points if you’re the first clinic mentioned, 1 point if you’re in the list but not first, 0 if absent.
- Accuracy (0, 1, or 2): 2 points if the AI describes you correctly (right treatments, right tone, no wrong info), 1 point if partially right, 0 if it’s wrong or says nothing.
Maximum is 6 points per check, so 216 points across all 36. Convert to a percentage: your points divided by 216.
Here’s how I read the score:
- 70 percent or higher: Strong AI visibility. Protect it and expand into new treatment queries.
- 40 to 69 percent: Inconsistent. You show up for some prompts or platforms but not others. This is the most common band and the most fixable.
- Below 40 percent: AI mostly doesn’t know you exist. This is where the biggest, fastest wins live.

A real example of what you’ll see
When I ran this for a clinic (I’ll keep them anonymous), the pattern was textbook. On prompt 1, “best medspa in [city],” they scored 0 on all three platforms. Nowhere to be found. But on prompt 11, “Is [Clinic] a good place for Botox,” all three AIs described them accurately and positively.
That gap tells a clear story: the reputation data was fine, but the discovery signals were missing. The AIs knew good things about them once prompted, but nothing was surfacing them in the first place. The fix wasn’t reputation management, it was getting cited in the “best of” listicles, directories, and structured content that AI models draw from for discovery prompts. You can’t diagnose that gap without running both the discovery prompts and the direct-name prompts. That’s the whole reason the script has 12 and not 3.
What to do with your score
Once you’ve scored yourself, three moves cover most of the gap:
- Fix accuracy first. If prompts 11-12 show wrong treatments or outdated info, correct your Google Business Profile, your website’s service pages, and your top directory listings. AI models trust structured, consistent data.
- Get into third-party lists. Discovery prompts lean heavily on “top medspas in [city]” articles. Earning a spot in even two or three of those changes your presence score fast.
- Publish treatment-specific content in a Q&A format. AI models quote clean, direct answers. A page that plainly answers “how much does lip filler cost in [city]” is far more quotable than a vague services page.
