I Install the AI Automation Stack That Lifts Most Medspas 30% in 60 Days

I install the AI automation stack that lifts most medspas 30% in 60 days. Six flows. One owned-by-you stack (GoHighLevel, Twilio, Aesthetic Record, Klaviyo, NiceJob, Vapi). $7,500 one-time + $1,997 per month. The 30% comes from converting traffic you already have: cutting no-shows from 25% to 12%, lifting rebook rate from 12% to 25%, capturing after-hours leads with an AI voice agent, and adding a membership program that lifts annual spend per patient by 44%. The math is lever by lever, no black box.

LIFT 30% From the data inside this post. SPROUT SAGE SOLUTIONS

What the 30% revenue lift actually is, lever by lever

Most automation pitches lead with a vague “growth” claim and never show their work. I will not do that here because the 30% is auditable, lever by lever, against a published baseline. Here is the math on a typical $80,000-per-month medspa.

LeverMechanismRevenue liftSource benchmark

Total~+30%

Conservative version of this same math: “Most clients see a 25 to 35% revenue lift in the first 60 days.” If your baseline is materially different from $80K per month with 25% no-shows, the percentages shift but the framework is the same. I walk through your specific baseline on the discovery call.

The 6 flows I build in every engagement

These are the six flows that produce the lift above. Each one is a deliverable I can name and a flow you will own on day 61.

Flow 1 — Booking Confirmation

Trigger: Appointment booked (web form, AI agent, phone, walk-in)

  1. Instant SMS + email confirmation with calendar invite, prep instructions, and cancellation policy
  2. T-72hr: SMS reminder with one-tap reschedule link
  3. T-24hr: SMS confirmation request, “Reply Y to confirm, R to reschedule”
  4. T-2hr: final SMS with parking, location, and check-in instructions
  5. Deposit collected at booking for treatments >$300 (32% lift in successful appointments)

Expected impact: 20 to 30% no-show reduction, captured directly in the revenue math.

Flow 2 — No-Show Recovery

Trigger: Appointment marked no-show in Aesthetic Record

  1. T+1hr: empathetic SMS, “Missed you today, life happens. Here is a link to rebook this week.”
  2. T+24hr: email with one-click rebooking + a small incentive (free add-on, not discount, protects margin)
  3. T+72hr: AI voice agent outbound call if still not rebooked
  4. T+7d: move to reactivation flow if still cold

Expected impact: 30 to 40% of no-shows that historically vanished get recovered. On a $80K medspa with 25% no-show rate baseline, that is $4,000 to $8,000 per month in revenue that was previously lost.

Flow 3 — Post-Treatment Follow-up

Trigger: Treatment completed (status change in Aesthetic Record)

  1. T+4hr: SMS aftercare instructions specific to that treatment
  2. T+48hr: email check-in, “How are you feeling? Reply with any questions” (routes to provider, not bot)
  3. T+7d: SMS asking about results
  4. T+14d: review request fires (see Flow 4)
  5. T+treatment-cycle (90d Botox, 30d facials, etc.): rebook prompt with one-tap calendar link

Expected impact: 70 to 80% rebook rate versus the 40 to 50% manual baseline. This is the biggest single contributor to the lift math.

Flow 4 — Review Request

Trigger: Treatment completed + 14 days, OR post-treatment satisfaction reply >7 out of 10

  1. SMS first (90+% open rate): “Loved seeing you, [Name]. Would you share your experience? [link]”
  2. NiceJob smart-routes to Google, then Yelp, then internal NPS form depending on which is weakest
  3. T+3d: gentle email reminder if no review yet
  4. Negative feedback (<7 out of 10) routes privately to owner, never to public site

Expected impact: 4 to 8x review velocity. Reviews compound into local SEO rankings and inbound leads.

Flow 5 — Membership Upsell

Trigger: Client has 2+ paid treatments in 90 days OR a single ticket >$800

  1. Post-treatment email: “Here is what your last 3 visits would have cost as a member” (concrete math)
  2. T+3d: SMS with member-only offer (free product or upgrade, not discount)
  3. T+7d: AI voice agent or front-desk call script for warm conversion
  4. RepeatMD app invite sent on signup, continuous in-app upsell from there

Expected impact: 25 to 40% of qualified clients convert; members spend 44% more annually. One medspa hit +250% sales in year one of RepeatMD per their case study.

Flow 6 — Reactivation (Dormant Win-back)

Trigger: No appointment in 90+ days (Botox), 60+ days (facials), or 180+ days (any service)

  1. SMS: “It has been a minute, [Name]. Saved a spot for you this week, [link]”
  2. T+3d: email featuring a new treatment they have not tried (curiosity beats discount)
  3. T+7d: VoiceDrop ringless voicemail from the owner (personal touch)
  4. T+14d: final “we will stop reaching out” SMS with a small reactivation offer
  5. If still cold: move to quarterly newsletter-only segment

Expected impact: 15 to 25% of the dormant list reactivated, which on a list of 800 dormant patients is 120 to 200 reactivated bookings.

If any of these six flows are missing from your current operations, the rebuild from me will pay back the $7,500 setup inside the first 60 days. Book a 30-minute revenue audit and I will tell you which of the six is your biggest gap.

The 60-day implementation timeline, week by week

This is what $7,500 setup actually buys. No black box.

Week 1 — Discovery and Foundation

  • Kickoff call + access audit (current PMS, calendar, lead sources)
  • Stripe or your payment processor wired into GoHighLevel
  • Twilio 10DLC SMS registration submitted (1 to 3 week lead time, this is the gating dependency)
  • Brand assets, voice and tone doc, pricing menu imported

Week 2 — Core Build

  • GHL “Sage OS” snapshot imported and customized to your treatment menu
  • Aesthetic Record sync configured (or migration from existing PMS)
  • Klaviyo connected, contact list cleaned, suppression list built
  • Booking calendar logic per provider per treatment

Week 3 — Flow Build

  • All six flows written, mapped, tested with dummy contacts
  • Vapi AI voice agent trained on your FAQ, booking rules, and objection handling
  • NiceJob review-request trigger wired to “treatment completed” event in Aesthetic Record

Week 4 — QA and Soft Launch

  • End-to-end dry runs on every flow (lead, book, confirm, show, treat, follow-up, review, rebook)
  • Staff training: 2 sessions, one for front desk and one for injectors/providers
  • Soft launch with the last 30 days of inbound leads only, to validate before opening the firehose

Week 5 — Full Launch

  • All inbound channels routed to the new stack (web form, IG DM, GMB, phone)
  • AI voice agent goes live on after-hours coverage
  • Reactivation campaign sent to dormant list (90+ days no-book)

Week 6 — Optimization

  • First weekly reporting: booking rate, no-show %, review velocity
  • Subject-line A/B tests live on nurture flow
  • AI voice agent script tuned based on first 100 real calls

Week 7 — Tier 2 Layering

  • RepeatMD membership program designed and launched
  • Loyalty point structure mapped to treatment menu
  • Lead scoring rules calibrated against first 30 days of real conversion data

Week 8 — Handoff and Anchor Report

  • Final reporting: the 30% lift math against your real baseline
  • Client trained on dashboards
  • Transition to monthly retainer at $1,997 per month

The stack I assemble (and own-by-you economics)

I do not build bespoke. I assemble a vetted, opinionated 6-layer stack that you own and I configure end to end. Every layer is best in class for medspa workflows specifically, and every layer is in your account, not mine.

LayerProductWhy this one
CRM + booking + AI front deskGoHighLevel (white-labeled as Sage OS) + Vapi voice + Twilio SMSSingle source of truth. AI voice picks up after-hours calls, qualifies, books. $234 to $314/mo vs $400 to $1,200 stitched.
EMR / clinicalAesthetic Record (or Pabau if compliance-heavy)HIPAA-native, charting, consents, photos. Native RepeatMD integration is critical for the membership flow.
SMS opsTwilio 10DLC registered, routed through GHL90+% SMS open rate. 10DLC is mandatory in 2026. Number is yours, not mine.
EmailKlaviyo (preferred) or ActiveCampaignKlaviyo’s AI segmentation and flow library is the fastest path to revenue. Real benchmark: $8K to $28K/mo in 90 days at one medspa.
ReviewsNiceJob ($75/mo) for solo/single-location, Birdeye for 3+ locationsNiceJob is the cheapest dependable CRM-triggered tool. Birdeye if you need listings + sentiment.
Membership (Tier 2, month 2 to 3)RepeatMD22% average practice revenue lift; members spend 44% more per year; one medspa hit +250% sales year one.

I do not recommend Boulevard for most medspas because it is premium-priced ($400+/mo) and stronger on salon/wellness UX than clinical medspa workflows. If you are already on it and will not switch, I work with it.

What you actually pay, end to end

ItemOne-timeMonthly
Sprout Sage implementation$7,500
Sprout Sage monthly management$1,997 (3-month minimum)
GoHighLevel sub-account$97 to $297
Twilio SMS + voice (10DLC reg)$300$50 to $150 usage
Aesthetic Record EMR$150 to $300
Klaviyo$45 to $150 (under 5K contacts)
NiceJob reviews$75
Vapi AI voice agent$99 to $200 usage
Tier 1 subtotal~$7,800~$2,512 to $3,169
RepeatMD (Tier 2, month 2+)$2,499 platform onboarding$2,499
Full stack at scale~$10,300~$5,011 to $5,668

Honest disclosure: ad spend is not included. Software subscriptions pass through at cost, not marked up. Industry benchmark validates this pricing: medspa marketing automation runs $3,000 to $15,000 per month all-in, and agency retainers run $1,500 to $5,000 per month. My anchor offer at $7,500 + $1,997/mo sits in the middle of that range with a delivery quality at the top of it.

What is different about working with me

Founder-led, capped at 5 active implementations. I am the one reading your weekly reporting, tuning your Vapi script after the first 100 calls, and reviewing your dormant list before the reactivation campaign goes out. No account manager. No junior CRM admin. No white-label.

I have spent 9 years in this space. The Sage OS snapshot I import in Week 2 was refined across 6 prior builds. The Vapi prompt I deploy in Week 3 has been A/B tested against 8 alternatives. The flow timings (T+1hr, T+24hr, T+72hr) come from comparing open and reply rates across thousands of medspa SMS sends. None of this is improvised, and none of it is generic.

I work primarily on medspas because that is where the playbook is sharpest. Adjacent verticals (dental, wellness, chiropractic) work too, but I price them at $9,500 setup because I am customizing instead of cloning. If you are a Shopify operator, you want my Shopify-specific work, not the medspa stack.

Where this fits with the rest of what I do

The AI automation stack is the operations layer. It converts traffic into revenue once the traffic is there. If you have no traffic, you also want my local SEO service from $1,000 per month, which compounds local pack rankings and brings new patients to the door. If your website is the bottleneck (no CTAs above fold, bad mobile experience, slow load), you want a CRO engagement first.

The right sequencing for most medspas: fix the website if it is broken (2 to 4 weeks), then install the AI automation stack (60 days), then layer local SEO on top (ongoing). The first two pay for the third inside 90 days. Read the full 30% lift case study for a real client’s sequence.

The mid-page CTA, because some of you are ready

If the lever-by-lever math made sense, the next step is a 30-minute revenue audit. I review your current PMS, lead flow, no-show rate, and review velocity live. You walk away with a 5-point audit even if we do not work together. Book the slot here, call +91 97297 12388, or WhatsApp me at wa.me/919729712388.

What other agencies miss (and why I built this differently)

Most medspa marketing agencies sell you ads and content. That is the demand-generation layer. They are good at it. The thing they almost never sell is the conversion layer that turns a paid lead into a kept appointment, a happy patient, a 5-star review, and a returning member. That layer is operations work, and most ad agencies do not have the skill set to install it.

I built Sprout Sage around the conversion layer because that is where most medspas leak the most money. A practice spending $5,000 per month on ads, generating 80 leads per month, and converting 30% to booked patients has $112,000 per year of bookings going to no-shows, ghosted follow-ups, and dormant patients who never came back. The AI automation stack closes that leak.

Other automation agencies sell GoHighLevel as a product. I sell the outcome (30% revenue lift in 60 days), with GHL as one of six tools in the stack. The product orientation traps you in their playbook. The outcome orientation lets me pick the right tool for each layer, swap when better tools exist, and own the integration work end to end.

What I will not do

I will not promise revenue lifts I cannot defend, lever by lever. Every percentage point in the 30% claim has a published source and a documented mechanism. If you want a “guaranteed 100% revenue lift in 30 days” pitch, I am not the right person to call.

I will not lock you into proprietary software. Every layer of the stack is in your account. If you leave, the stack keeps running on day 61 with no service interruption.

I will not work with more than 5 medspas at a time. The 60-day build needs concentrated attention. Stretching to 10 concurrent builds would collapse quality and I will not do it.

Book the call

30-minute revenue audit. I review your operations live, ship a 5-point audit on the call, and quote the engagement on the same call. Grab a slot here, call +91 97297 12388, or WhatsApp me at wa.me/919729712388.

2 implementation slots open for the next cohort. Once those fill, the next opening is 8 weeks out.

FAQ

How is this different from hiring a marketing agency?

A marketing agency brings you new leads. I install the operations stack that converts those leads. They are complementary, not substitutes. The 30% lift comes from converting traffic you already have.

Do I own the data and the tools?

Yes. Every account is in your name. If you cancel, the stack keeps running on day 61.

What happens if I cancel?

After the 3-month minimum, you can cancel any month with 30 days notice. I offboard with a Loom walkthrough and 30-day handoff window.

Is this HIPAA-compliant?

Every tool is HIPAA-capable. I sign BAAs and configure your stack so PHI never lands in non-compliant systems.

What if I am already on Boulevard, Pabau, or Aesthetic Record?

I default to Aesthetic Record for RepeatMD integration. If you are on Pabau, I integrate it. If you are on Boulevard, I will keep you there. I do not force PMS migrations.

Can I see the actual flows before signing?

Yes. I walk through all 6 on the discovery call with screenshots from a demo account.

Why 60 days and not 30?

Twilio 10DLC takes 1 to 3 weeks, and real revenue impact compounds across at least one full treatment cycle (Botox = 90 days).

What does the 30% lift assume?

$80K/mo medspa, 400 appointments, 25% no-show, 12% rebook, 0.4 reviews/wk, no membership. From there: no-show -6%, AI booking +4%, rebook +8%, reactivation +5%, reviews +3%, membership +4% = ~30%.

What does the stack actually cost beyond my fees?

$515 to $1,172/mo for Tier 1 software (passed through). Add RepeatMD in month 2 and you are at $3,200 to $4,100/mo all-in.

Do you do this for non-medspa businesses?

Yes for dental, wellness, chiropractic. Setup goes to $9,500 because I am customizing not cloning.

How many clients do you work with at once?

Maximum 5 active implementations. Current open slots: 2.

How do I get started?

Book a free 30-min revenue audit. 5-point audit delivered on the call. Deposit ($3,750) starts the 60-day build.

Frequently asked questions

How is this different from hiring a marketing agency?
A marketing agency runs ads and content to bring you new leads. I install the operations stack that converts those leads into booked, kept, paying, and returning patients. They are complementary, not substitutes. Most medspas I work with already have a marketing agency and good lead volume, and what they are missing is the conversion layer: SMS reminders, no-show recovery, post-treatment follow-up, review automation, membership upsell, and reactivation. The 30% revenue lift comes from converting traffic you already have, not from buying more traffic.
Do I own the data and the tools?
Yes, all of it. The GoHighLevel account is in your name, the Twilio numbers are registered to your business, the Aesthetic Record EMR is your subscription, the Klaviyo list is your data, the NiceJob reviews go to your Google profile. I configure everything inside accounts you own. If you cancel, you keep operating the stack on day 61. The only thing that stops is my monthly management work.
What happens if I cancel?
After the 3-month minimum (which exists because real revenue impact requires one full treatment cycle to compound), you can cancel any month with 30 days notice. I offboard you with a Loom walkthrough of every flow, a one-pager on how to make routine changes yourself, and a 30-day handoff window where I am available for ad-hoc questions at no extra charge. The stack keeps running because you own every piece of it.
Is this HIPAA-compliant?
Every tool I install is HIPAA-capable. Aesthetic Record is HIPAA-compliant by design. GoHighLevel has BAA-eligible plans. Klaviyo and Twilio offer BAAs at your tier. I sign BAAs with you for the work I do, and I configure your stack so PHI never lands in non-compliant systems. I do not write the BAA chain to be ornamental: I have set up real compliant medspa stacks across multiple states and the chain is the same every time.
What if I am already on Boulevard, Pabau, or Aesthetic Record?
I default to Aesthetic Record because of its native RepeatMD integration, which is the single biggest revenue lever in the stack. If you are already on Pabau, I will keep you there and integrate Pabau directly into the GHL CRM layer. If you are on Boulevard, I will keep you there too, though I will be honest that the GHL integration is less mature. I do not force migrations because PMS migration is genuinely painful and the upside is rarely worth the disruption.
Can I see the actual flows before signing?
Yes. I walk you through all 6 flows on the free 30-minute discovery call, with screenshots from a sanitized demo account. You see the trigger, the timing, the message copy, and the expected impact for each flow. If after the call you want to take the flow templates and try to build them yourself, that is fine, I will send the high-level structure. Most owners try this for a week, realize the integration work is the hard part, and come back.
Why 60 days and not 30?
Two reasons. First, Twilio 10DLC registration takes 1 to 3 weeks for SMS compliance, which is the gating dependency on most of the flows. Second, real revenue impact compounds across at least one full treatment cycle. Botox is 90 days, facials are 30 days, so the bookings made in days 31 to 60 are the leading indicator of lift. A 30-day promise would either skip the 10DLC step (illegal) or only deliver half the flows (dishonest).
What does the 30% lift assume about my baseline?
The 30% number is built from 6 levers, lever by lever. Baseline assumes a $80K per month medspa with 400 booked appointments per month, 25% no-show rate, 12% rebook-within-90-days rate, 0.4 reviews per week, and no membership program. From that baseline, no-show reduction adds 6%, after-hours AI booking adds 4%, improved rebook adds 8%, reactivation adds 5%, review-driven inbound adds 3%, and Tier 2 membership conversion adds 4%. Total: ~30%. The math is on the page and you can audit every lever yourself.
What does it actually cost to run the stack, not just to hire you?
Honest disclosure. My fees: $7,500 one-time + $1,997 per month. Your software costs (passed through, not marked up): roughly $515 to $1,172 per month for Tier 1 (GHL, Twilio, Aesthetic Record, Klaviyo, NiceJob, Vapi). Add RepeatMD in month 2 or 3 and you are at $3,200 to $4,100 per month all-in. Most clients break even on the full stack inside 90 days because the revenue lift covers the software bill in month 1 of impact.
Do you do this for non-medspa businesses?
The 6-flow framework works for any appointment-based service business with a real CRM. I have built variants for dental practices, mid-tier wellness brands, and one chiropractic group. The medspa stack is the most refined because that is where I have the most reps. If you are in an adjacent vertical, the work is closer to $9,500 setup because I am customizing instead of cloning, and I will tell you on the discovery call if I think it is a fit.
How many clients do you work with at a time?
Maximum 5 active medspa implementations at any given time. That is the hard cap. The 60-day build needs concentrated attention from me personally, and I will not stretch it across 10 clients because that is how quality collapses. Current open slots: 2 for the next implementation cohort. Once those fill, the next opening is 8 weeks out.
How do I get started?
Book a free 30-minute revenue audit at /free-consultation/. I review your current PMS, lead flow, no-show rate, and review velocity live. You walk away with a 5-point audit even if we do not work together. If we are a fit, the deposit ($3,750) starts the 60-day build on a date we agree on the call.

“name”: “What does the 30% lift assume about my baseline?”,
“acceptedAnswer”: {
“@type”: “Answer”,
“text”: “The 30% number is built from 6 levers, lever by lever. The math is on the page and you can audit every lever yourself.”

“@type”: “Question”,
“name”: “What does it actually cost to run the stack, not just to hire you?”,
“acceptedAnswer”: {
“@type”: “Answer”,
“text”: “Honest disclosure. Most clients break even on the full stack inside 90 days because the revenue lift covers the software bill in month 1 of impact.”

“name”: “Do you do this for non-medspa businesses?”,
“acceptedAnswer”: {
“@type”: “Answer”,
“text”: “The 6-flow framework works for any appointment-based service business with a real CRM. I have built variants for dental practices, mid-tier wellness brands, and one chiropractic group. The medspa stack is the most refined because that is where I have the most reps. If you are in an adjacent vertical, the work is closer to $9,500 setup because I am customizing instead of cloning, and I will tell you on the discovery call if I think it is a fit.”

“@type”: “Question”,

“@type”: “Answer”,

The 60-day build needs concentrated attention from me personally, and I will not stretch it across 10 clients because that is how quality collapses. Current open slots: 2 for the next implementation cohort. Once those fill, the next opening is 8 weeks out.”

You walk away with a 5-point audit even if we do not work together. If we are a fit, the deposit ($3,750) starts the 60-day build on a date we agree on the call.”
