Orthopedic Marketing: In-House vs Agency

Choosing in-house or agency marketing for an orthopedic practice starts with the work that needs an owner. A new hire, an agency retainer and a hybrid arrangement solve different capacity problems. Before comparing fees, list the tasks, the expertise required and the hours your practice can actually make available.

This worksheet is for owners and administrators deciding how to resource marketing. It does not assume that outsourcing is cheaper or that hiring guarantees better control. Sprout Sage Solutions offers orthopedic agency services; the comparison below helps you decide which responsibilities you need covered before discussing a provider.

Compare three working arrangements

ArrangementWhere it may fitQuestion to resolve
In-houseThe practice can recruit and manage the expertise needed for recurring work.Who covers specialist tasks and absences, and how will work be reviewed?
AgencyA defined external scope can address a capacity or expertise gap.Which deliverables are included, who performs them and what still needs practice input?
HybridAn internal owner coordinates priorities while specialists handle agreed tasks.Who makes decisions and resolves work that falls between teams?

Assign the recurring tasks

Use the following list in a planning meeting. Give each item one accountable owner, the person doing the work, an approver and an available time allowance. A role title alone does not establish capacity.

  • Approve priorities across surgeons, service lines and locations.
  • Supply accurate physician credentials, office information and service availability.
  • Prepare and review website changes and internal links.
  • Review search visibility and maintain approved local information.
  • Draft content and arrange the practice’s clinical or operational approval.
  • Check enquiry delivery with the receiving staff.
  • Maintain a change log and explain reporting limitations.
  • Coordinate suppliers, access requests and handovers.

Keep clinical approval, service availability and scheduling decisions with the practice’s designated team. An agency can support the communication and implementation process; it cannot establish those underlying facts on the practice’s behalf.

Use your own cost inputs

Compare the same period and scope. For an internal role, include the compensation and employment costs your organization uses, recruitment, tools, specialist support and management time. For an agency, include fees, media, software, development and the practice time needed for approvals. For a hybrid, include both sets without counting shared costs twice.

Do not compare one employee’s salary with a retainer that covers a different set of tasks. Our orthopedic marketing scope and budget guide can help separate the cost categories. Use actual proposals and your own staffing assumptions rather than generic salary estimates.

A project may be enough

If the immediate problem is a broken enquiry route or a small set of incomplete pages, ask whether a bounded project can resolve it. A continuing engagement needs recurring work, named responsibilities and a reporting process that supports decisions. For organic search work, review the orthopedic SEO service scope before assuming a full marketing retainer is required.

Turn the decision into a brief

Record what your team will retain, what you want a supplier to deliver and how the teams will coordinate. The orthopedic agency RFP template turns those decisions into questions that bidders can answer consistently. If you already have offers, compare them with the proposal worksheet.

Discuss the marketing capacity your orthopedic practice needs. Share your website, your role and the work your current team cannot cover. We will discuss fit before proposing responsibilities or fees.
