Why this operation surprises people
Hip arthroscopy patients skew young. They are athletes, runners, and active people in their twenties and thirties, and that changes the financial picture in two specific ways.
They are far more likely to be on a high-deductible plan, so a larger share of the bill genuinely lands on them rather than on an insurer.
They are also more likely to be told the procedure is not medically necessary. Insurers scrutinise FAI surgery closely, prior authorization is common, and denials happen often enough that the appeal should be planned for rather than reacted to.
The bill, itemised
The facility. The largest and most variable line. An ambulatory surgery center is usually well below a hospital outpatient department for identical work, and hip arthroscopy is commonly an outpatient procedure, so this choice is often genuinely available to you.
Anchors and implants. Labral repair is anchor-driven. Two anchors and five anchors are different consumable bills, and the surgeon frequently cannot know the count until they are inside the joint.
The surgeon’s fee. Commonly est. $2,500 to est. $7,000. As always, a minority of the total.
Anesthesia. Usually est. $700 to est. $2,200. Hip arthroscopy requires traction and precise positioning, so operating times run longer than patients expect.
Physical therapy. This is the line that catches people. Rehabilitation after hip arthroscopy commonly runs three to six months. According to typical published outpatient rates, visits run est. $75 to est. $250 each, and a course of twenty or more visits is normal. That total can rival the out-of-pocket cost of the operation itself, and it appears in no surgical estimate.
Insured versus self-pay
In network, you pay deductible plus coinsurance to your out-of-pocket maximum, which for many 2026 commercial plans lands the episode somewhere around est. $2,000 to est. $8,000 before therapy.
Bundled self-pay quotes for hip arthroscopy at surgery centers commonly sit between est. $12,000 and est. $22,000. Ask whether anchors are inside the bundle or billed separately, because on this procedure that is the question that matters.
What to ask before you schedule
Ask whether the case can be done at a surgery center and what the facility fee is at each option. Ask how many anchors the estimate assumes and what happens if more are needed. Ask how many physical therapy visits are typical and what your plan pays per visit. Ask whether prior authorization is required and who handles an appeal. Ask for the anesthesia group’s name and check its network status yourself.
What this means if you run a sports medicine practice
This patient researches on a phone, compares three surgeons in an evening, and makes a decision partly on which practice was straightest with them about money.
I audited a sports medicine site last year with excellent anatomy and rehab content and nothing at all about cost. The front desk fielded price calls all day with no script. The single thing patients most wanted was the one thing the practice refused to put in writing.
Publishing cost does not mean publishing your fee schedule.
It means naming the four bills, giving honest ranges labelled as estimates, saying out loud that the facility choice matters, and being specific about the physical therapy tail. Being the practice that warns someone about the rehab cost before surgery rather than after is worth more trust than any amount of advertising.
Then handle prior authorization visibly. A practice that says plainly how it deals with denials is answering the fear this specific patient actually has.
