A lead is not a new patient, and a new patient is not a completed care plan.

Most chiropractic marketing is measured at the wrong point. An agency reports leads. Your front desk knows how many of those became a first appointment. You know how many finished care.

Those three numbers are wildly different, and only the last one pays for anything.

I measure at the end of that chain and work backwards, because a cheap lead that never shows up is the most expensive thing you can buy.

The number that actually runs a chiropractic practice

New patient acquisition cost, measured against what a completed care plan is worth. Not cost per lead, not cost per click, and not impressions.

The gap between those matters enormously. A practice with a $40 lead and a 20% show rate is paying $200 a new patient. A practice with a $90 lead and a 70% show rate is paying $129. The second one looks worse in every agency report and is significantly better in reality.

What I actually do

Local search, which is most of the battle

Chiropractic is overwhelmingly a proximity purchase. Google Business Profile done properly, categories and services complete, review generation built into your discharge process, and service-area pages that are real rather than templated.

Pages for the conditions people actually search before booking

Sciatica, lower back pain, neck pain, headaches, sports injury, pregnancy care, auto accident. Each is a different person with a different worry. One services page covering all of them ranks for none of them.

The first appointment, not the lead

Online booking that works on a phone, honest expectations about the first visit, and a clear answer on cost and insurance before they call. Most of the drop-off happens between interest and appointment, and it is fixable with words rather than budget.

Tracking that ends at the appointment

Call tracking, form tracking, and where your practice software allows it, a way to tie a booking back to its source. Reporting that stops at leads is reporting that cannot improve anything.

On claims, because this category has rules

Healthcare advertising is regulated and chiropractic claims get scrutinised more than most. I will not write copy promising to cure conditions, and I will push back if asked to. That is partly ethics and mostly self-interest on your behalf: a complaint costs more than any campaign earns.

The things you are actually thinking, answered

"This sounds like the cold emails I delete every day"

It should, and that is a reasonable reaction. The people who own this category have trained you to expect a pitch with no substance. The difference I can offer is checkable: before any call I look at your site and your search visibility and tell you what I would fix first, whether or not you hire me. If that conversation is not specific to your business, you will know inside five minutes and you should end it.

"I am not paying for someone offshore to do this badly"

I am based in India and I work with clients in the United States, the UK and Israel. Saying that plainly is better than letting you find it out later. What it means in practice: my afternoon and evening covers the whole UK working day and the US morning, so you get same-day replies rather than overnight ones. What it does not mean is a team of juniors. You get me, which is also the limit, because I take a small number of clients at once.

"The last agency took my money and I got nothing"

This is the most common thing I hear and usually the story is the same: a long contract, a monthly report full of numbers that went up while the phone did not ring, and no way to leave. So there is no contract here. You can stop at the end of any month, and the site, the content, the tracking and the ad accounts are yours from day one, in your name, not mine.

"How do I know you will not just send me traffic that never converts?"

Because traffic is not what gets reported. The events that matter are phone taps, form submits and booking clicks, tracked from the first week so that by month two we can both see which pages produce enquiries and which produce visits. If a page brings people and no enquiries, that is a finding, not something to bury under a sessions chart.

"I do not have time to manage another supplier"

You should not have to. What I need from you is roughly an hour a month, plus answers when something needs a decision only you can make. If a project needs more of your time than that, I will say so before it starts.

Who you deal with: me, every time. There is no account manager and no handover to a junior, which is the advantage and also the reason I keep the client list short.

What happens if it is not working: you leave at the end of any month and keep everything. I would rather you did that in month three than stay unhappy for a year, because the referrals I get from people who left on good terms are worth more than one uncomfortable retainer.

What it costs

A single-doctor practice filling two more slots a day and a three-location group opening a fourth need very different work. Anyone quoting a monthly figure before knowing which you are is guessing.

So the first call is me understanding your business, not me presenting a package. Then I come back with a scope and a monthly figure built for that.

What I will tell you upfront: it is a monthly retainer, there is no contract, you can stop any month, and everything I build stays yours. If we run ads, that spend goes to Google in your own account and I do not mark it up.

Being straight about proof

I have not run a chiropractic account, and I will not dress up an adjacent one as though I had.

What is genuinely relevant: I work with orthopedic and physical therapy practices, which share the buyer, the local search dynamics and the referral patterns almost exactly. The strongest result I can show you is a patient-facing cost comparison page that produced a significant share of one client's enquiries, plus a local search rescue that moved a service business from position seven to position two in the map pack over 90 days. What is a realistic new patient acquisition cost for a chiropractor?

It varies enormously by market and by whether you are cash pay or insurance based, so any agency quoting you a universal number is guessing. The useful approach is to measure your own: total marketing spend divided by new patients who actually attended, then compare it against your average completed case value. That ratio tells you what you can afford.

Do you guarantee new patient numbers?

No. Nobody controls Google, and patient volume depends on your reviews, your location, your front desk and your availability as much as on marketing. What I commit to is named deliverables monthly, tracking from day one, and no contract.

What does chiropractic marketing cost?

Should I market cash pay or insurance?

Whichever you actually want more of, and they need separate messaging. Cash-pay patients are choosing a provider and respond to expertise, outcomes and convenience. Insurance patients are often choosing based on coverage and location. Mixing both messages on one page usually converts neither well.

How important are Google reviews for a chiropractor?

Close to decisive for local pack position, and heavily influential on whether someone calls you rather than the practice next door. Volume and recency both matter. The most valuable change is usually asking at the right moment in the care cycle rather than asking more often.

Can you make claims about what chiropractic treats?

Not the kind some practices would like. Healthcare advertising is regulated and this category attracts scrutiny, so I will not write copy promising to cure conditions. Well-written honest copy converts perfectly well and does not expose you to a complaint.

How long before I see new patients from this?

Local search usually shows first movement in roughly 60 to 90 days, as an estimate rather than a promise. Changes to your booking flow and your review process can show up faster than that, which is why I usually start there.
